New Patient Forms
Before your visit, we will email or text a link and a password for your online forms. Complete them ahead of time so we can focus on you when you arrive.

If you prefer to complete forms at the office, arrive at least 15 minutes before your appointment.

We accept most medical plans, including Blue Cross/Blue Shield, Medicare, Aetna, UnitedHealthcare, and many more.
Medical insurance may apply when we evaluate or treat an eye health problem. Bring both your medical and vision cards.
